we have been experiencing a merging of these two sessions (morning / afternoon) which causes inefficiencies and we are trying to re-introduce this definition. for more information please click e-consult welcome to our new, exciting and easy to use service. try this service for routine medical enquiries. to get started, just click on the image below. please note you will receive a response via email . unless otherwise advised, your econsultation will result in either a telephone consultation or a face to face appointment with a clinician. please note this service cannot currently be used to request sick/fit notes . please print off a fit/sick note request from this website. the form can be found under "further information" at the bottom of this page. click on "sick certificate", then the link under the heading "medical sick certificate". complete the form and drop it in to the surgery. we regret we cannot accept requests for prescriptions using this method. please sign up to our online service (details below) or visit the surgery. for nursing appointments please contact reception. we sincerely regret the dpa breach that occurred in late january this year. this has been reported to the information commissioners office and they have successfully concluded their investigations. an internal review has also been conducted. we continue to develop a safe and secure platform for all patient communications. online services follow this link to sign in to online services. for some time you have been able to book and cancel appointments and order repeat prescriptions online. you have also been able to see some basic information from your medical record, including your medication history, immunisations and allergies. you are able to request access to more of your medical record, including diagnoses, test results and procedures. this is known as your detailed coded record (dcr). access may be granted at your doctor's discretion. for more information and guidance when applying for access to your dcr, please see our patient information leaflet or click here for nhs guidance. to register for online services, each family member who wishes to register must visit either surgery with a current form of id, preferably photographic (driving licence or passport), and complete a form available in reception. those patients who currently have access to online services and wish to request access to their dcr will need to re-register and provide id as above. due to the issues surrounding proxy access (i.e. access to online services by somebody e.g. a parent acting on behalf of the patient), we are currently only registering adults (aged 16 and over) for online services. however we may be able to allow proxy access to parents/guardians of under 11 year olds to include booking appointments, requesting prescriptions, and viewing immunisations, allergies and medication history in the near future. should you encounter any difficulties when registering or accessing your online account, please follow this link . patient participation group the aim of the patient participation group (ppg) is to work in partnership with the living well partnership and to strengthen the relationship between the practices and patients. the ppg is open to all patients registered at all practices within the living well partnership and all living well partnership staff. for more information about the living well partnerships ppg please click here . please see the noticeboards in waiting rooms for details of the next living well partnership ppg meeting which are rotated between sites. new members are always welcome. drop into your surgery for more information should you wish to come along or join. friends & family test the friends and family test is a way for all patients to give feedback quickly and have their say on the care and treatment they experience across the nhs in england. it helps to improve local healthcare services for everyone by highlighting what is working well and what can be improved. the fft has been in use in some hospital services since 2013 and is now expanding to more parts of the nhs, including gp practices from december 2014. for full details, see www.nhs.uk/friendsandfamily all living well partnership surgeries are able to offer you the opportunity to participate in this survey. you can pick up a survey card the next time you visit your surgery, you can use the ipads -
